Job summary

AltaPointe Health is seeking staff to support adult outpatient programs, performing assessments, education, and crisis de-escalation in a clinical setting. The role requires adhering to policies, documentation, and patient-centered care with a focus on safety and quality outcomes.

Applicants should have a high school diploma or equivalent and be at least 23 years old with a valid driver license. Training in mental health provisions will be provided within six months of hire.

Qualifications

• Possess a High School Diploma or equivalent.
• Must be at least age 23, maintain a valid verifiable driver license and a clear driving record.
• Knowledge in mental health service provision, to be attained within six months of employment.

Responsibilities

• Assess consumers for deficits as determined by the DLA-20.
• Instruct patients on how to improve abilities in deficit areas.
• Demonstrate and assist consumer in performing tasks to evaluate comprehension.
• Educate patient on continuing to implement the skills they have acquired.
• Maintain established institutional policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
• Demonstrate ability to recognize crisis elements and deescalate or resolve situations.
• Collaborate with supervisor to ensure steps needed to engage patients are met and patient is set up for success.
• Collaborate with treatment team members and regularly provide feedback on patient status.
