Qualifications

Responsibilities

• Advocate for clients and ensure access to all services per the treatment plan.
• Coordinate and document resources and services for each client.
• Maintain weekly contact with youth, families, and agencies/parents.
• Facilitate communication among treatment team members.
• Coordinate admissions, discharges, and transfers of caseload.
• Assist with paperwork, assessments, and data entry into the electronic health record.
