Responsibilities

• Advocates for the client and ensures the client receives all needed services and resources.
• Facilitates communication among team members and with external parties.
• Engages in crisis intervention with youth.
• Coordinates admissions, discharges, and transfer of caseload.
• Meets with youth, parents, and guardians to complete paperwork and enter client data into the EHR.
