Job summary

Pala Band Of Mission Indians is seeking a Case Manager in the Social Services department to provide trauma-informed, crisis-oriented case management for adolescents and transitional age youth in danger.

Under supervision, you will assess needs, develop individualized service plans, coordinate referrals, and connect clients with housing, healthcare, and recovery supports while maintaining HIPAA-compliant records.

Qualifications

• Bachelor’s degree or equivalent in related field; advanced degree preferred.
• Minimum two years of case management or crisis services experience.
• Experience working with Native American communities preferred.

Responsibilities

• Conduct comprehensive assessments to identify behavioral health, medical, social service, and recovery support needs.
• Develop individualized service plans with participants and families.
• Coordinate behavioral health services, referrals, and community resources.
• Facilitate warm handoffs between Lifeline and providers, hospitals, and schools.
• Monitor progress and update service plans to promote stability and recovery.
• Maintain HIPAA-compliant documentation and participate in team meetings.
• Travel within the service area as needed.
