Covenant House California is seeking a Time Limited Subsidies / Transitional Living Program Case Manager to deliver comprehensive case management to youth aged 18-24. You will coordinate ISPs, housing navigation, and aftercare, while maintaining HMIS and collaborating with CHC and external partners.

The role emphasizes trauma-informed practices, crisis intervention, and strong communication to ensure safe transition to stable housing.

Competenze

• Minimum three years of case management experience, preferably with homeless youth.
• Bachelor's degree in social services or related field from an accredited college or university.
• Bilingual English/Spanish preferred.

Mansioni

• Provide case management services to youth in the Transitional Living Program (TLP) and Time Limited Subsidies Program (TLS).
• Develop and implement Individual Service Plans (ISPs) and track progress monthly.
• Conduct intake and assessments within 3–7 days; support housing navigation and lease coordination.
• Maintain accurate documentation in HMIS/ETO and coordinate with partner agencies.
• Attend team meetings, group activities, and trainings as required.
