The Sacramento LGBT Community Center in Sacramento, CA is seeking a STEP Case Manager to support transitional-aged youth in a 90-day shelter program. You will build 90-day case plans, coordinate housing, education, employment readiness, and mental health support on-site at the shelter and at the Center’s Lavendar Heights location.

You will maintain documentation in our client database and HMIS, conduct crisis intervention, advocacy, and resource navigation, and collaborate with housing

Qualifications

• Knowledge of LGBTQIA2S+ youth needs and intersections with race and diversity.
• Trauma-informed, client-centered approach to case management.
• Ability to work independently and in a team environment.
• Proficiency in English is required.

Responsabilités

• Provide case management for transitional-aged youth in homelessness.
• Crisis intervention, resource navigation, advocacy, housing support, and employment readiness.
• Document accurately in client databases and HMIS per agency guidelines.
• Collaborate with housing advocates, peer supports, and mental health professionals.
