Job summary

Community Teamwork is seeking a Housing Support Case Manager at the Youth Opportunity Center in downtown Lowell to assist at-risk and homeless youth ages 14-25 with housing stabilization and well-being goals. The role emphasizes advocacy, coordination with community resources, and data-driven case management.

Qualifications

• High School diploma or equivalent education.
• 2+ years of experience working with at risk youth.
• Reasonable ability to work with at risk youth in group and individual settings.
• Minimum of one year assessment, advocacy and case management in Human Services.
• Knowledge of community resources for low-income individuals and families.
• Excellent customer service, organizational, oral and written communication skills.

Responsibilities

• Assess housing needs with each participant.
• Formulate individualized housing and stabilization plans.
• Perform regular Youth Housing visits in the community.
• Identify strengths and address barriers from assessments.
• May support other Youth Services team members as needed.
