Job summary

Community Teamwork Inc. is seeking a Housing Support Case Manager to join the Youth Services team at the Youth Opportunity Center in downtown Lowell.

The role focuses on case management for at-risk and homeless youth aged 14–25, providing advocacy, support and connections to services to achieve housing stability and well-being. The position emphasizes building individualized service plans, conducting community visits, and collaborating with partners to address barriers to self-sufficiency,

Qualifications

• High School diploma or equivalent education.
• 2 years of experience working with at risk youth.
• Minimum of one year assessment advocacy and case management work in a Human Service field.
• Demonstrated knowledge of community resources available to low income individuals and families.

Responsibilities

• Assess for general housing needs with each participant.
• Formulation of housing and stabilization individualized service plans.
• Perform regular Youth Housing programming visits in the community.
• Work closely with individuals to identify and build upon strengths and develop strategies to address barriers identified through the assessment process.
• May assist other Youth Services team members as needed supporting programming and difficult or complex situations.
