Job summary

Open Sky Community Services in Worcester, MA is seeking an Aurora Youth Program Navigator to provide intensive, person-centered case management and direct outreach for about 25 young adults living in SRO housing.

You will collaborate with housing partners, behavioral health and educational services to promote stability, independence, and successful transition to independent living; duties include housing navigation, benefits access, employment and education support, and coordinating referrals.

Qualifications

• High School Diploma, GED, or equivalent required.
• Valid driver's license with reliable transportation required.
• Minimum of two years' experience working with or lived experience with homelessness or related populations.
• Bachelor's degree in social work, psychology, or related field preferred.

Responsibilities

• Engage residents through regular, person-centered interactions to promote trust and independence.
• Provide housing navigation and support to promote stability and tenancy.
• Assist with housing documentation, certifications, and applications.
• Support residents in employment, education, and benefits access.
• Coordinate referrals and maintain confidentiality.
