Job summary

Open Sky Community Services is seeking an Aurora Youth Program Navigator in Worcester, MA to provide intensive, person-centered case management and direct outreach for approximately 25 young adults in SRO units.

The role emphasizes housing navigation, skill-building for independent living, and coordinating with partners to ensure access to education, employment, and community resources. Position is full-time and on-site.

Qualifications

• High School Diploma, GED, or equivalent.
• Valid Driver’s License with acceptable driving history and reliable transportation.
• Minimum of two years’ experience with homelessness, justice involvement, behavioral health needs, substance use disorders, or underserved populations.
• Bachelor’s degree in social work, psychology, human services, or related field preferred.

Responsibilities

• Engage residents through regular, person-centered interactions that promote trust and independence.
• Provide ongoing housing navigation and support to promote stability and tenancy.
• Assist residents with housing documentation, applications, and recertifications.
• Support residents in developing independent living skills and connecting with community resources.
• Coordinate referrals when needs exceed Navigator scope and maintain confidentiality.
