Job summary

YMCA of San Diego County is seeking a Social Work Case Manager for the Housing Our Youth (HOY) program in City Heights. This in-person role supports youth experiencing homelessness with case management, referrals, and access to healthcare, education, and employment resources.

The position emphasizes trauma-informed, culturally responsive care, with a schedule Monday–Friday, 9 a.m.–5 p.m., and may involve transporting participants in YMCA vehicles as needed. Role ends 6/30/2027.

Qualifications

• Bachelor's degree in social science, psychology or related field; up to one year of related work experience.
• Knowledge or experience of providing social work practices to individuals and families.
• Basic computer knowledge and skills.

Responsibilities

• Assess and plan with each participant; coordinate with participants and collaborating agencies; provide services, advocacy and referrals.
• Conduct participant-screening interviews and assess service needs in various settings.
• Employ critical thinking, problem solving and conflict management during crisis response.
• Facilitate activities to develop life skills and support employment and education opportunities.
• Maintain concise case notes and organized records for all participants.
