Job summary

Washington University Youth HIV Clinic in the United States is seeking a Case Manager II to provide client-centered HIV case management for youth ages 13–29, including comprehensive bio-psychosocial assessments and care coordination with a multidisciplinary health care team.

You will develop individualized care plans, coordinate referrals, document contacts, and support education on HIV/AIDS, confidentiality, and transition to adult care, while engaging with families and community partners.

Qualifications

• Master's degree or combination of education and/or experience may substitute for minimum education.
• LMS/HRIS or similar systems experience preferred.
• Experience with HIV/AIDS education and youth services is a plus.

Responsibilities

• Conduct comprehensive bio-psychosocial assessments with youth and families.
• Develop individualized plans of care and coordinate service referrals.
• Maintain regular contact with clients and document care coordination activities.
• Educate clients and families about HIV/AIDS and medication adherence.
• Coordinate access to internal and community resources per guidelines.
• Participate in multidisciplinary team meetings and quality improvement.
