Job summary

Washington University in St. Louis is seeking a Case Worker II to provide client-centered HIV case management and thorough bio-psychosocial assessments in coordination with a multidisciplinary healthcare team.

The role develops and implements care plans, coordinates referrals and benefits, delivers crisis intervention and supportive counseling, and ensures confidentiality in accordance with state and federal guidelines.

Qualifications

• Master’s degree or combination of education and/or experience may substitute for minimum education.
• Basic Life Support certification must be obtained within one month of hire date.
• No other specific work experience is required for this position.

Responsibilities

• Conducts a comprehensive assessment with the client and family to develop a plan of care.
• Arranges and coordinates service referrals and monitors plan implementation; refers to public benefits as eligible.
• Documents all contacts and coordination activities in client records as required by program guidelines.
• Educates clients/families about basic medical and psychosocial aspects to optimize adherence.
• Provides proactive follow up, tracks symptoms and treatment responses.
• Adheres to state laws and department regulations; stays current with resources.
