Job description

Position Summary

The position of Case Worker I is intended to provide client-centered HIV case management services and thorough bio-psychosocial assessment for individuals in coordination with the multidisciplinary healthcare team. The Case Worker I will create and implement multidisciplinary care plans, identify educational needs, refer and coordinate community resources, provide crisis intervention and supportive counseling, and coordinate access to resources in accordance with all applicable local, state, and federal guidelines.

Scheduled Hours

40 hours per week.

Job Description

Primary Duties & Responsibilities:

  • Conduct comprehensive bio-psychosocial culturally-sensitive assessments with clients and their support network. Synthesize program goals and assessed needs to create individualized plans of care.
  • Arrange and coordinate service referrals; monitor activities and evaluate progress on care plans. Refer to public benefits programs as eligible.
  • Manage caseload with regular contact and documentation in client records as required by program guidelines.
  • Educate clients and families about medical and psychosocial aspects of HIV/AIDS to optimize adherence to care.
  • Adhere to state laws and department rules regarding HIV/AIDS programs.
  • Maintain current knowledge of medical and psychosocial developments concerning HIV/AIDS and network with ASOs, providers, clinicians, and other service providers.
  • Collaborate with case managers from other regions as needed.
  • Perform indirect, non-clinical tasks such as data compilation, training participation, and attendance at staff and community meetings as assigned.
  • Participate in multidisciplinary HIV Specialty Health Care Team conferences to coordinate care.
  • Educate clients/families on medication adherence and safer sex practices.
  • Work collaboratively within Adult ID Clinic multidisciplinary team and the Project ARK social services team.
  • Attend monthly Ryan White community-wide case management meetings.
  • Assist with other client support activities as deemed appropriate.

  • Typical sitting at desk or table; repetitive wrist, hand or finger movements; occasional lifting up to 25 lbs.

Education & Qualifications

Required Qualifications: Bachelor’s degree in a health-related field (social work, counseling, sociology, psychology) or equivalent with case management experience.

Preferred Qualifications: None unless stated elsewhere.

Certifications & Licenses

Basic Life Support certification (American Heart Association or American Red Cross) must be obtained within one month of hire. Online BLS certifications without a skills assessment component are not sufficient.

Grade & Salary

Grade: C08-H.

Salary Range: $20.57 - $30.84 per hour.
