Job summary

Qualifications

• Basic Life Support certification must be obtained within one month of hire date.
• Bachelor’s degree in social work, counseling, sociology, or psychology.
• Experience in case management is preferred.

Responsibilities

• Conduct comprehensive assessments and create individualized care plans.
• Coordinate service referrals and monitor implementation of care plans.
• Educate clients on HIV/AIDS and support adherence to treatment.
