Qualifications

• Master’s degree in social work, public health, or related field.
• Current Basic Life Support (BLS) certification from AHA or Red Cross.
• No specific work experience required for this position.

Responsibilities

• Conduct comprehensive assessments to develop care plans.
• Coordinate referrals and monitor service delivery.
• Educate clients/families on medical and psychosocial aspects.
• Maintain confidential client records per guidelines.
• Support long-term goals and coordinate home-based care when needed.
