Job summary

Sweetser is seeking a Care Coordinator for High-Fidelity Wraparound in Saco, ME. The role emphasizes family-driven, youth-guided planning and cross-system coordination to support children and families in community settings.

You will collaborate with youth, families, and multiple service systems, ensuring individualized, strengths-based care plans and timely documentation within an accredited program. A bachelor’s degree and driver’s license are required.

Qualifications

• Bachelor’s degree in a field related to psychology, mental health, or human services.
• Completion of Wraparound training within 15 months of hire.

Responsibilities

• Establish and maintain partnerships with youth and caregivers using family-driven, youth-guided principles.
• Engage youth and families as equal decision-makers in planning and service coordination.
• Coordinate services across behavioral health, education, child welfare, and medical providers.
• Develop and update wraparound plans of care with family voice and natural supports.
• Maintain timely documentation in the EHR in compliance with policy and regulatory standards.
• Provide services in community, home, school, and other environments including evenings.
