Rate: $22 hourly
Care coordinators provide support to individuals accessing the Community Behavioral Health Clinic [CCBHC], through supporting intake/access to care and ongoing monitoring of care and needs. Care coordinators manage ongoing coordination of care for a caseload of individuals to include facilitating ongoing collaboration among providers and linkage to needed services.

ESSENTIALFUNCTIONS:
- Responsiblefor the coordination of care of individuals in the Community BehavioralHealth Clinic [CCBHC].
- Facilitateongoing collaboration and communication between providers.
- Supportsthe intake/access processes through completion of risk screening andbehavioral health/SUD screenings with individuals.
- Linksindividuals with needed resources and services and regularly ensuresservices are meeting needs.
- Completetimely and accurate documentation of all contacts and activities utilizingan Electronic Health Record.
- Completescomprehensive surveys with individuals at intake, discharge and every sixmonths while in care.
- Responsiblefor tracking and administering surveys as well as data entry.
- Conductsprimary care screening and monitoring processes and procedures.Coordinates vaccinations for Hepatitis A and B series for clients, asindicated by screening procedures.
- Supportindividuals in completion of applications for applicable benefits toinclude Mainecare, non-emergency transportation resources, and othersocial services.
- Utilizesupervisory supports appropriately and participates in all team/programmeetings.
- Otherduties as assigned.

- Maintainscurrent knowledge in present practice area, i.e., attendance at requiredhours of organization educational programs and utilizes clinicalsupervision as a learning experience.
- Uselistening skills, flexible collaboration and a consistent problem-solvingfocus in working to resolve differences with colleagues, peers andclients.
- Communicateideas and opinions in a clear and constructive manner, both verbally andin writing.
- Patientfacing position - mental health experience 1-2 years preferred.
