Together for Youth in Melville, NY is seeking a Community Care Specialist to provide intensive one-to-one interaction and support to youth and families through recreational, educational, cultural and social activities. You will assist with supervising, counseling, teaching and coordinating services, and you will collaborate with providers to ensure best service delivery.

A valid driver’s license and ability to travel are required.

Formación

• Minimum High School diploma required.
• Associate’s degree preferred.

Responsabilidades

• Provide individual treatment and supportive services to youth and families.
• Develop and implement individual service plans.
• Conduct family team meetings to discuss ongoing decision making.
• Advocate for youth with community agencies and link to resources.
• Maintain regular communication with guardians, foster parents, and biological parents.
• Assist with youth supervision and transportation as needed.
• Collaborate with service providers (e.g., DSS, Mental Health Clinic).
• Document status and progress on goals and complete reports in a timely manner.
