Job summary

Together for Youth in Melville, NY is seeking a Family Specialist to coordinate comprehensive case management to safeguard youth in foster care and advance permanency and well-being. A BA in Human Services is required; MSW preferred forclinical roles.

You will partner with families, schools, and providers to implement treatment plans and pursue reunification when possible. The role requires a flexible schedule (8:30 AM–5 PM, some evenings), a valid driver's license, and reliable vehicle for home

Qualifications

• Bachelor's degree in Human Services, Social Work, Psychology, or related field is required.
• Master's degree (MSW, MHC, MFT or related) is strongly preferred for clinical/supervisory roles.
• 1 year of direct experience with at-risk youth and families is preferred.
• Flexibility to work days, evenings, and occasional on-call responsibilities.
• Valid driver's license and access to a reliable vehicle for frequent travel.
• Ability to coordinate transportation for home visits and court obligations.
• Ability to engage diverse families with equity and inclusive practices.
• Excellent communication and accurate, timely written documentation.
• Proficiency in Microsoft Office and web-based documentation systems.

Responsibilities

• Conducts assessment of family strengths and needs and utilizes treatment resources to promote growth.
• Develops and evaluates comprehensive treatment and discharge plans, identifying alternative placements when needed.
• Delivers case management services, schedules counseling, and ensures medical/dental exams are completed.
• Maintains timely, confidential documentation and adheres to consent and information release standards.
• Performs home and community casework visits at least twice monthly to support treatment goals.
• Acts as a liaison between youth, family, and foster care teams; attends treatment reviews and meetings.
• Supports reunification through permanent supports, mentorship, after-school activities, and relapse prevention planning.
