Job summary

Together for Youth in Melville, NY seeks a Community Care Specialist to provide intensive one-to-one support to youth and families through education, recreation, and social activities. The role involves supervising, counseling, teaching and coordinating with the care team to connect families with community resources.

The successful candidate will have a High School diploma (Associate’s preferred), at least 1 year working with youth, a valid driver's license, and flexibility for evenings,

Qualifications

• High School diploma required; Associate’s degree preferred.
• Minimum 1 year experience with youth and families preferred.

Responsibilities

• Provide one-to-one support to youth and families through education, recreation, cultural and social activities.
• Develop individual service plans and participate in family team meetings.
• Advocate for youth with community agencies and link to resources.
• Maintain communication with guardians and foster/biological parents; provide transportation as needed.
• Collaborate with DSS and Mental Health Clinic to improve service delivery.
• Document progress on goals and complete reports in a timely manner.
