This full-time, non-exempt role supports waitlisted youth with short-term case management and timely documentation in a trauma-informed, strengths-based framework.

Responsibilities include scheduling, coordinating with treatment teams, risk assessments, and facilitating community partnerships and monthly gatherings.

Qualifikationen

• Bachelor's degree with relevant experience.
• Experience facilitating events, team meetings and trainings.
• Ability to assess needs, strengths and provide crisis intervention and de-escalation.
• Passionate about working with young adults who have experienced complex trauma.
• Values teamwork, cultural humility, compassion, inclusivity and social justice.

Aufgaben

• Track and manage youth referrals across electronic health records and spreadsheets.
• Provide short-term case management and crisis resources for waitlisted youth.
• Obtain clinical documentation to inform care planning.
• Organize program orientations for new referrals.
• Engage providers to champion team-based care across service phases.
• Monitor regulatory compliance in care plans and Medi-Cal documentation.
• Cultivate community partnerships and plan monthly gatherings.
• Uphold mission and participate in supervision and professional development.
