Job summary

WestCoast Children's Clinic is seeking a Mental Health Care Coordinator for high needs youth in Oakland. The role combines in-person and virtual service delivery within the C-Change program, focusing on intensive care coordination and family-driven planning.

You will manage up to 19 clients with crisis support, documentation, and collaboration with a multidisciplinary team to achieve individualized goals for youth and families.

Qualifications

• Group, training or treatment team facilitation experience and skills are required.
• Strong crisis assessment and intervention skills.
• Ability to travel within a 90 mile radius of the WCC office.
• Flexible schedule with evenings and occasional weekends.

Responsibilities

• Assess and coordinate intensive behavioral health and care plans for high-acuity youth.
• Lead and document Child & Family Team (CFT) meetings with stakeholders.
• Work with a multidisciplinary team to support goals and progress.
• Maintain detailed progress notes and crisis intervention when needed.
• Provide services in a mix of in-person and virtual settings.

Education

Bachelor's or Master's degree in Psychology, Social Work, Human Services, Counseling, or related field
