Job summary

WestCoast Children's Clinic in Oakland, CA seeks an Outpatient Intensive Care Coordinator (ICC) to assess, coordinate, and oversee intensive behavioral health plans for youth with complex needs. You will convene Child & Family Teams, provide crisis intervention, and collaborate with a multidisciplinary group in both in-person and virtual settings.

The role requires 2+ years in youth/family mental health, a relevant degree, and the ability to travel within the Bay Area.

Qualifications

• Minimum 2 years of direct work with children and/or families in mental health or related services.
• Group, training or treatment team facilitation experience required.
• Bachelor's or Master's degree in Psychology, Social Work, Human Services, Counseling, or related field is required or considered with relevant experience.
• Ability to travel within ~90 miles of Oakland while maintaining a valid CA license/insurance.

Responsibilities

• Conduct comprehensive biopsychosocial and behavioral health assessments; develop and monitor individualized care plans.
• Organize and facilitate Child & Family Team (CFT) meetings with youth, families, and stakeholders.
• Collaborate within a multidisciplinary treatment team to support youth and families toward goals.
• Manage a caseload and ensure 60% of time is dedicated to direct behavioral health services; document thoroughly.
• Assess crises and make safe, ethical decisions; provide risk management and referrals as needed.
