Job summary

Integrated Services for Behavioral Health is seeking an Engagement Care Coordinator to support youth involved in the OhioRISE program. The role focuses on discharge planning, educating families and partners about OhioRISE, and coordinating follow-up care and services.

The position involves attending in-person and virtual meetings, developing care plans, and facilitating Child and Family team meetings until a smooth handoff to ongoing care coordination occurs.

Qualifications

• Experience providing services to children and families in behavioral health or related public sector fields.
• Minimum education requirements as listed in the job posting (HS diploma or higher; advanced degrees preferred).
• Familiarity with Hi-Fidelity Wraparound or willingness to obtain certification during employment.

Responsibilities

• Identify care coordination needs with families to support children, youth, and families.
• Coordinate family-based services in home, school, and community.
• Engage with families and teams to develop and implement care plans and safety plans.
