Job summary

Integrated Services for Behavioral Health is seeking an Engagement Care Coordinator to support youth hospital discharge within the OhioRISE program in Pickaway County, Ohio. The role involves coordinating care, educating families and partners about OhioRISE, and arranging follow-up services with warm handoffs to ongoing coordinators.

The position emphasizes collaboration with families, schools, and community resources, plus attendance at in-person and virtual meetings to complete engagement

Qualifications

• Experience providing services to children and families connected to behavioral health or related public sector human services fields.
• Three years with a high school diploma or equivalent; or two years with an associate or bachelor's; or one year with a master's.
• Hi-Fidelity Wraparound knowledge preferred (Certification provided at time of employment).
• Two years of experience in a coordinated supportive services or care coordination role preferred.
• Experience working with autism spectrum disorders and developmental disabilities preferred.
• Be culturally humble or responsive with training and experience to manage complex cases.
• Excellent organizational skills and strong cultural awareness.
• Must have a valid Driver License and be able to operate in an Internet-based office environment.

Responsibilities

• Joins with family to identify care coordination needs and align with program outcomes.
• Works with families to define cultural factors influencing engagement and care planning.
• Identifies strengths of children, youth, and families for care coordination engagement.
• Coordinates family-based services in home, school, and community.
• Ensures services on care plans are appropriate, least restrictive, and safe.
• Engages with families and teams to support integration of care plans.
• Develops partnerships with community resources to meet diverse needs.
• Maintains documentation, participates in program evaluation and meetings.
• Keeps up on training requirements including High Fidelity Wraparound and cultural humility.
• Performs other duties as assigned.
