Job summary

Integrated Services for Behavioral Health is seeking an Engagement Care Coordinator to support youth hospitalized under the OhioRISE program. You will assist in discharge planning, education of families and partners, and coordinate follow-up care with community resources.

Responsibilities include leading team meetings, developing care plans, and ensuring services are appropriate and accessible. A background in pediatric behavioral health and strong communication are essential.

Qualifications

• Experience providing services to children, youth and families in behavioral health or related public sectors.
• Ability to coordinate care plans, crisis safety, and family meetings.
• Knowledge of OhioRISE program and community resources is a plus.
• Strong communication and documentation skills.

Responsibilities

• Assist in immediate care coordination for youth hospitalized under OhioRISE.
• Educate families and partners about the OhioRISE program.
• Schedule and facilitate child and family team meetings and warm handoffs.
• Maintain thorough documentation and participate in program evaluations.
