Integrated Services for Behavioral Health in Ohio is seeking an Engagement Care Coordinator to support youth in the OhioRISE program, assisting with discharge planning, education, and referrals. You will participate in meetings, develop care plans, crisis safety plans, and coordinate family-centered services across home, school, and community settings, ensuring culturally humble practice.

Qualificações

• Minimum of three years with a high school diploma or equivalent, or two years with an associate or bachelor’s degree, or one year with a master’s degree or higher.
• Experience in care coordination or coordinated supportive services preferred.
• Experience with behavioral health, child welfare, or related public sector human services.
• Familiarity with Hi-Fidelity Wraparound preferred (Certification provided at time of employment).

Responsabilidades

• Identify care coordination needs and services with families to meet program outcomes.
• Define cultural factors influencing family engagement for ongoing success.
• Coordinate family-based services in home, school, and community.
• Ensure services on care plans are appropriate, least restrictive, and safe.
