Responsibilities

• Identify care coordination needs with families to meet program outcomes.
• Engage families to define cultural factors influencing care planning.
• Coordinate home, school, and community-based services for youth and families.
• Ensure services on care plans are appropriate and meet safety needs.
• Participate in team meetings and warm handoffs to ongoing coordinators.
