Job summary

Integrated Services for Behavioral Health in Ohio seeks an Engagement Care Coordinator to assist with care coordination for youth enrolled in the OhioRISE program. You will support hospital discharge, educate families and partners about OhioRISE, and arrange follow-up referrals.

The role involves attending meetings to complete engagement milestones, creating crisis safety plans, care plans, and coordinating Child and Family team meetings until warm handoff to an ongoing care coordinator.

Qualifications

• Experience providing services and/or support to children and families connected to behavioral health or related public sector human services.
• Three years with a high school diploma or equivalent; or two years with an associate or bachelor's; or one year with a master's degree or higher.
• Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment).
• Two years of experience in a coordinated supportive services or care coordination role preferred.

Responsibilities

• Joins with family to identify care coordination needs/services in line with service delivery standards and program outcomes.
• Works with families to define cultural factors influencing strengths and family interactions.
• Coordinates family-based-services for children, youth, and families in home, school, and community.
• Engages and builds positive relationships with children, youth, and families to support care plans.
• Develops partnerships with community resources to meet diverse needs of youth and families.
• Maintains documentation, participates in program evaluation, attends team meetings, and participates in cross-systems training.
