Job summary

Southwest Network seeks a dedicated Case Manager to support children, young adults, and families. You will facilitate the Child and Family Team process, helping develop individualized service plans and connect clients with community resources.

You will coordinate care, document progress, and collaborate with families and providers to promote wellness and stable housing, school engagement, and positive outcomes for youth.

Qualifications

• Must possess one of several behavioral health degree paths.
• Must maintain CPR, First Aid, and Therapeutic Options certifications.
• Must have Arizona driver's license and car insurance; 21+.
