Southwest Network is hiring a Case Manager to work with children, teens and families in Arizona. You will coordinate the Child and Family Team process to develop individualized service plans and connect youths with community resources.

The role emphasizes collaboration with families, educators and health providers, ensuring integrated behavioral and physical health care. Strong communication, organization, and caregiving skills are essential.

Qualifications

• Must possess one of the specified degrees in behavioral health or related field.
• CPR, First Aid, and Therapeutic Options certifications required.
• Arizona fingerprint clearance card required.
• Arizona driver’s license and car insurance required.
• Must be at least 21 years old.

Responsibilities

• Facilitate the Child and Family Team process for youths with complex needs.
• Develop individualized service plans and link to community resources.
• Coordinate behavioral and physical health care.
• Partner with families and others involved in the youth’s life.
