Job summary

Southwest Key Programs seeks a Case Manager to complete assessments and service plans, enabling a comprehensive case file in SWK’s web-based system. You will maintain organized schedules, handle evening/weekend on-call needs, and provide advocacy and home visits as required.

The role requires building therapeutic relationships with clients and families, coordinating with schools and community partners, and ensuring documentation is completed promptly across the program.

Qualifications

• Bachelor’s degree in Behavioral Sciences, Human Services or related field with two years’ experience with at-risk youth and families.
• Must be computer literate with Windows Word/Excel and online databases.
• Flexible schedule to meet program services; strong written and verbal communication.
• Able to work independently and as part of a team; pass pre-employment background check; valid TX driver’s license.

Responsibilities

• Conduct initial intake with client and family.
• Ensure timely completion of intake paperwork, assessments and service plans.
• Maintain client files and document in SWK database within 48 hours of service.
• Provide crisis intervention and home visits as needed.
• Coordinate with schools and community partners for client progress.
