Job summary

Southwest Network is seeking a dedicated professional to facilitate Child and Family Team meetings and assess youths using developmental tools. The role involves home visits and community outreach to support foster families and care providers.

You will collaborate with caregivers, group homes, and shelters to identify needs and link youths with essential services, ensuring holistic care.

Qualifications

• Must hold one of the listed education/experience combinations.
• Must maintain CPR, First Aid, and Therapeutic Options certifications.
• Must have Arizona fingerprint clearance card.
• Must have a valid Arizona driver’s license and car insurance.
• Must be at least 21 years old.

Responsibilities

• Facilitate or assist in facilitation of Child and Family Team (CFT) meetings.
• Conduct observations and assessments of youths using developmental/mental health tools.
• Engage with youths and families in homes and community settings to assess needs.
• Maintain regular contact with foster families, caregivers, group homes, and shelters.
