Job summary

Integrated Services for Behavioral Health (ISBH) in Fayette, OH is seeking a Care Coordinator to support youth and families under OhioRISE. You will lead child and family team meetings and coordinate services across home, school, and community settings to promote family stability and reduce hospitalizations.

The role involves collaboration with social service providers, ensuring HIPAA compliance, and maintaining accurate documentation.

Qualifications

• Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector human services.
• Two years in a coordinated supportive services or care coordination role with high efficiency, organization, and attention to detail.
• Experience with autism spectrum disorders and developmental disabilities is preferred.

Responsibilities

• Develop, lead, and facilitate team meetings for the young person and their identified team.
• Provide care coordination within the family’s home, community, or preferred location.
• Partner with the young person, family, natural supports, and other service providers to identify care coordination needs/services.
• Maintain documentation to access and maintain OhioRISE benefits.
• Travel within assigned service area.
