Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a dedicated Care Coordinator for the OhioRISE program in Fairfield County, OH. This role centers on creating, planning, and organizing a team to support youth at home, in school, and in the community, ensuring voice and choice in coordinated care.

The position emphasizes collaboration with families, social services, and partners, maintaining documentation, and meeting OHRISE standards.

Qualifications

• Experience coordinating services for youth with behavioral health needs.
• Knowledge of Medicaid billing processes and care coordination.
• Ability to communicate effectively with families, providers, and community partners.
• Proven ability to manage documentation and maintain HIPAA compliance.

Responsibilities

• Develop, lead, and facilitate team meetings for the youth and family.
• Provide care coordination in home, school, or community settings.
• Collaborate with families, natural supports, and service providers to identify needs and goals.
• Maintain required documentation to access and sustain OhioRISE benefits.
• Travel within the assigned service area and participate in trainings.
