Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordinator to organize and lead care planning for youth within the OhioRISE program. The role supports children at home, in school, and in their communities, ensuring voice and choice in services while coordinating with families and providers.

The position emphasizes teamwork, HIPAA compliance, and efficient documentation in EHR systems, with travel to various community sites as needed.

Qualifikationen

• Experience with behavioral health services for children and families
• Knowledge of OhioRISE or Medicaid services is a plus
• Ability to work in home and community settings
• Effective communication and time management skills
• Experience in coordinating multi-agency teams
