Job summary

Red Rock Behavioral Health Services in Chandler, OK seeks a Care Manager I-Level I to implement comprehensive care plans addressing mental health, physical health, and life goals. Qualifications

• BHCM I certification required or equivalent
• Independent judgment and interpersonal skills
• Completion of in-service and external training requirements
• Valid Oklahoma Driver License with clean MVR and insurance
• 2+ years of experience with community services and health agencies
• Commitment to Red Rock Health mission

Responsibilities

• Provides advocacy, coordination, side-by-side support, problem solving, and direct assistance to help consumers obtain daily living necessities.
• Participates in disposition/treatment team meetings and contributes relevant treatment information.
• Helps develop, write, implement, and revise Service Plan goals and objectives.
• Coordinates services and communicates with DHS or referral sources; advocates for treatment and reintegration plans.
• Educates and supports consumers’ families.
• Ensures Primary Care engagement and supports self-management of physical health; completes paperwork and authorizations as required.
• Documents consumer progress for permanent records and maintains confidentiality.
• Provides ongoing assessment of mental health symptoms and adjusts treatment plans as needed; ensures compliance with Medicaid, ODMHSAS, CARF, and other funding sources.
• Demonstrates knowledge of co-occurring disorders and applies training.
• Assists with coordination for community resources and linkages to self-help programs; maintains partnerships with local/state/federal agencies.
• Promotes PTSD and crisis awareness in the community; supports care plans within Red Rock Health Team.
• Performs other duties as required
