Job summary

Milwaukee County is seeking a Crisis Care Coordinator (Youth) within Behavioral Health Services to work with youth and their families to identify and access community resources. Responsibilities

• Meet with youth and families weekly to provide direct services until enrolled or linked to wraparound supports.
• Develop individualized care plans measurable under state guidelines and document progress notes.
• Collaborate with other providers and coordinate services as needed to support DHHS and Wraparound operations.
• Perform other duties as assigned.

Salary Pay Range: $49,753.60 - $74,630.40

Job Summary: TheCrisis Care Coordinator (Youth) will work with youth and their families to identify and access community-based resources. Position is to follow up and/or meet face to face with families in the community within two business days of the referral to review programs, obtain consents, and create an individualized plan for the time between referral and enrollment into Wraparound programs. This role will also connect youth and their families to any community support and services needed.

Essential Responsibilities

• Deliver Crisis Care Coordination until other services can be obtained or the youth has stabilized. This may include assistance with benefits, linkage to other services, discussion of stressors, crisis stabilization services, etc.
• Create a communication plan with families to meet weekly face to face. Work in collaboration with youth and family to create a crisis plan that documents how a crisis will be responded to and identify what the family’s needs are for linkage to support and services. Meet with youth in ineligible settings and participate in court and discharge plans.
• Meet with youth and family on a weekly basis (or more if needed) to provide direct services to the individual until enrolled in programming or connected to non-Wraparound community supports.
• Develop individualized care plans that are measurable under DHS 34 state guidelines. Document direct services via progress notes promote a youth's recovery goals and meet all documentation requirements as per state guidelines.
• Collaborate with other service providers and coordinate services as needed.
• Contribute to redesign planning and implementation of new initiatives and other tasks to support the Department of Health and Human Services (DHHS), Behavioral Health Services (BHS), and Wraparound operations.
• Perform all other duties as assigned.

Minimum Requirements

• Current Resident of Wisconsin.
• Possession of a valid Wisconsin Driver’s License and insured personal vehicle, both able to be maintained throughout employment.
• Possession of a bachelor’s degree or higher Human Services, Psychology, Social Work.
• At least one (1) year of professional experience with providing mental health services to young children or adolescents.
• Intermediate knowledge ofall related computer and software applications, such as word processing and spreadsheets.
• Willing and able to travel up to 75% of the time in this role.

Preferred Qualifications

• Possession of LPC or LCSW licensure.
