Job summary

Zepf Center in Lucas County, Ohio, seeks a full-time Youth Community Based Case Manager to provide community-based support for individuals with serious mental illness and/or substance use challenges. You will coordinate care, advocate for clients, and link to community resources, working across home, community, and agency settings.

The role emphasizes person-centered planning, cultural responsiveness, and collaboration with a multidisciplinary team to improve emotional and physical health

Qualifications

• HS diploma or higher with three years of relevant experience.
• Bachelor's or Master's degree in a related field preferred.
• Strong communication and documentation skills required.

Responsibilities

• Assess clients and develop comprehensive care plans with the team.
• Maintain contact and support clients to attain goals and outcomes.
• Refer clients to community resources and coordinate services.
• Advocate for clients while coordinating with agencies and families.
• Document interactions in the electronic health record within timelines.
