Job summary

Zepf Center, a leading behavioral health provider in Lucas County, seeks a full-time Substance Use Disorder Case Manager to coordinate wraparound care for adults in outpatient programs. You will deliver counseling, advocacy, and resources to support recovery and coordinate with a multidisciplinary team in office, homes, and community settings.

Qualifications

• Experience in behavioral health settings.
• Knowledge of DSM-5 disorders.
• CDCA certification required at hire.

Responsibilities

• Provide wraparound integrated care coordination for adults with SUD/MH in outpatient programs.
• Deliver client-centered counseling, case management, and linkage to services.
• Develop and update strength-based Individualized Action Plans (IAP) every 90 days.
• Coordinate referrals and access internal/external resources.
• Document admissions, progress notes, IAP reviews, and related records.
• Monitor relapse risk and changes in level of care; adjust plans accordingly.
