Job summary

Zepf Center in Northwest Ohio is seeking a full-time Substance Use Disorder (SUD) Case Manager to support adults in SUD/MH treatment programs. You will deliver wraparound care coordination, advocacy, and counseling in office, home, and community settings.

Responsibilities include developing Individualized Action Plans every 90 days, documenting admissions, progress, and releases, facilitating referrals, and supporting recovery through motivational techniques.

Qualifications

• 3 verifiable years in behavioral health OR a related Bachelor's degree.
• Working knowledge of DSM-5 diagnoses.
• Must have CDCA certification in good standing through OCDPB at hire.

Responsibilities

• Provide wraparound integrated care coordination and advocacy for adult clients.
• Develop and update an Individualized Action Plan (IAP) every 90 days.
• Document admissions, discharges, progress notes, IAP reviews, and reports.
• Facilitate referrals and linkages to internal and external resources.
• Support clients' recovery processes and provide counseling.
• Promote relapse prevention and healthcare management.
