Job summary

Jefferson County in Wisconsin is seeking a CCS Service Facilitator I for Youth to deliver psycho-social rehabilitative services to youth with mental health or substance use concerns within the CCS program.

You will coordinate service plans, assess progress, collaborate with families and schools, and support recovery teams while pursuing evidence-based practices such as CBT and DBT. A bachelor’s degree and driver’s license are required; wage starts at $31.45–$35.06 per hour.

Qualifications

• Bachelor’s degree in social work or related field.
• At least one year of related experience with children, adolescents and families.
• Experience delivering or participating in evidence-based models.
• Valid Driver’s License.
• Must pass the caregiver background check.

Responsibilities

• Facilitates and coordinates psychosocial rehabilitative services as specified in the CCS Array, including screening assessments, service planning, service facilitation and follow-up care with consumers and their recovery team.
• Monitors and supports physical health and wellness management as part of integrated service delivery.
• Ensures consumer participation in the team-developed service plan and supports consumers with identifying, working toward, and achieving goals.
• Completes initial and ongoing assessments within required CCS timelines to support service planning and evaluate consumer progress.
• Collaborates with both consumer and recovery teams to collect information about consumers’ family mental health and substance use histories and provides recommendations for treatment and services.
