Job summary

Professional Services Group in Jefferson County, WI, is seeking a Service Facilitator for the CCS Service Facilitation program. The role provides intensive case management for children and adults with severe emotional disturbance, operating within a wraparound service delivery model and coordinating with case managers, providers, school staff, and other community partners to ensure coordinated care.

This full-time position requires a Bachelor's or Master's degree, experience with SED, reliable

Qualifications

• Must have reliable transportation on a daily basis, a valid driver's license, and automobile insurance.
• Bachelor's Degree in Social Work, Psychology, Criminal Justice, or other related fields.
• Minimum of one year of experience working with the severely emotionally disabled population (SED).

Responsibilities

• Create individualized treatment plans for each assigned client.
• Develop a family support team comprised of formal and informal supports.
• Facilitate team meetings with service providers, family members, and family supports to monitor progress.
• Develop a crisis/safety plan for the client and family.
• Advocate for clients in emergency and crisis situations.
• Attend client/family IEP meetings, psychiatry appointments, court proceedings, or other appointments requiring family support or advocacy.
• Safely transport clients as needed.
