Job summary

Milwaukee Succeeds is hiring a CCS Service Facilitator I in Jefferson County to provide psycho-social rehabilitative services to youth consumers with mental health and/or substance-use disorders. You will facilitate services guided by individualized recovery plans and support consumers' emotional and functional stability.

This role requires a Bachelor’s degree in social work or a related field and at least one year of relevant experience. Wage starts at $31.45–$35.06 per hour, depending on qualifications, with comprehensive benefits such as health insurance and retirement plans.

Qualifications

• One year of related experience in a clinical or community-based setting.
• Experience with evidence-based models.
• Valid Driver’s License required.

Responsibilities

• Facilitate and coordinate psychosocial rehabilitative services.
• Monitors physical health and wellness management.
• Ensure consumer participation in service plans.
• Complete assessments within CCS timelines.
• Collaborate with recovery teams for consumer treatment.

Skills

Experience with children and adolescents

Knowledge of trauma-informed care

Ability to teach evidence-based practices
