Job summary

AMIkids, Inc. is seeking a Case Manager to oversee service planning and transition processes for youth in our program. The ideal candidate will develop individualized plans, conduct intake assessments, and monitor progress within a multidisciplinary team to ensure effective outcomes.

A Bachelor’s degree and experience with youth are required, with opportunities for growth and comprehensive benefits. We offer health benefits, holidays, PTO, retirement plans and opportunities for advancement

Qualifications

• Bachelor’s degree in psychology, Social work or related field.
• Minimum 2 years’ experience working with youth.

Responsibilities

• Complete intake needs assessment, re-assessments, and transition planning.
• Evaluate, develop, and document an individual service/care plan for youth.
• Participate in Multidisciplinary Treatment Team (MDT).
• Deliver evidence-based treatment models with fidelity and proper documentation.
• Attend and represent the program at court hearings and youth proceedings.
