Job summary

AMIkids - Infinity Wake in Raleigh, NC seeks a Case Manager to oversee service planning and the transition process for youth in the program, ensuring a safe and productive learning environment that promotes social and emotional growth.

The role requires collaboration with the MDT, completion of needs assessments, and development of individualized service plans. Responsibilities include coordinating services, attending court proceedings, and maintaining thorough documentation.

Qualifications

• Bachelor degree in Psychology, Social work or related field.
• Minimum 2 years’ experience working with youth.
• Able to obtain crisis intervention and physical restraint training as defined by state requirements.
• CPR and First Aid Certification may be required.

Responsibilities

• Oversee service planning and transitional processes for youth in the program.
• Active participant in the Multidisciplinary Treatment Team (MDT).
• Complete intake needs assessments and re-assessments within contractual guidelines.
• Develop and document individual service/care plans based on needs assessments.
• Ensure timely delivery of program services to meet youth needs.
• Attend court hearings and youth-related meetings; represent the program.
