Job summary

AMIkids in Raleigh, NC seeks a Case Manager to oversee service planning and the transition process for youth, fostering a safe learning environment and supporting social and emotional growth through academic goals. You will participate in MDT, conduct intakes, and develop individualized care plans to ensure timely academic, vocational, and behavioral assessments.

Qualifications

• Bachelor degree in Psychology, Social Work or related field.
• Minimum 2 years' experience working with youth.
• Crisis intervention and physical restraint training and certification as defined by state requirements.
• May be required to obtain and maintain current CPR and First Aid Certification.

Responsibilities

• Actively participate in the Multidisciplinary Treatment Team (MDT).
• Complete intake needs assessment on entering youth and re-assessments as required.
• Develop and document individual service/care plans and updates.
• Ensure timely academic, vocational, and behavioral assessments within enrollment timelines.
• Deliver program services to meet needs per the Personal Growth Model and foster interpersonal skills.
• Attend court hearings and youth proceedings as needed.
• Deliver Evidence Based Treatment models and maintain fidelity and documentation.
• Facilitate youth progress meetings with MDT monthly or as needed.
• Schedule transition meetings 60 days before release and develop Transition Plan 30 days prior to termination.
• Ensure post-needs assessments and completed transition plans before release.
• Perform administrative tasks: monthly reports, weekly updates, audits, notes, and off-site appointment logistics.
