Job summary

AMIkids in Raleigh, NC seeks a Case Manager (Facility Based Day Treatment) to oversee service planning and the transition process for youth entering and exiting the program.

In this role you will participate in the Multidisciplinary Treatment Team, conduct intake assessments, develop individualized service plans, coordinate academic, vocational and behavioral evaluations, and ensure timely delivery of services while maintaining safety and compliance with program standards.

Qualifications

• Bachelor's degree in Psychology or Social Work or related field.
• Minimum 2 years’ experience working with youth.
• Crisis intervention and physical restraint training per state requirements.

Responsibilities

• Oversee service planning and transitional processes for youth in the program.
• Complete intake and periodic reassessments within contractual guidelines.
• Develop and document individual service/care plans for youth.
• Attend MDT meetings and participate in court hearings as required.
• Ensure timely delivery of program services and maintain case notes.
• Schedule transport and coordinate off-site appointments.
