AMIkids, Inc. is seeking a Case Manager to oversee service planning and transition processes for youth in our program. The ideal candidate will develop individualized plans, conduct intake assessments, and monitor progress within a multidisciplinary team to ensure effective outcomes.

We are seeking a Case Manager to oversee the service planning and transitional process for youth entering and exiting the program. The ideal Case Manager is excited to develop an individualized plan for each youth we serve while monitoring that plan to make sure it is effective. As a Case Manager, you will be complete intake needs assessment, re-assessments, and transition planning,
Evaluate, develop, and document an individual service/care plan for the youth based on the needs assessment, and complete required updates.
An active participant in the Multidisciplinary Treatment Team (MDT).
Deliver Evidence Base Treatment models adhere to fidelity requirements of evidence-based treatment model fidelity and documentation.
Attend and represent the program at court hearings, meetings, and youths’ legal proceedings.

Bachelors or better in Psychology or related field.
